Aged Care and Disability Services Coordinator
Position Overview
The Aged Care and Disability Services (ACDS) Coordinator is responsible for the day-to-day operational management of aged care and disability programs in the community, including the coordination of individualised client care and supervision of frontline staff. The role ensures high-quality service delivery that supports clients to live safely and independently, remain connected to community and culture, and access services in accordance with relevant legislation, funding requirements, and Groote Archipelago Regional Council (GARC) policies.

Key Responsibilities
- Coordinate the day-to-day operations of ACDS programs in the community, including staff rosters, resource planning, and service delivery logistics.
- Provide case management and client support, including development, review, and monitoring of individualised care plans.
- Supervise and support ACDS staff to deliver high-quality, person-centred services, including performance management, mentoring, and training.
- Ensure compliance with Aged Care Quality Standards, NDIS Practice Standards, GARC policies, and relevant legislation.
- Maintain accurate and timely records, including case notes, assessments, care plans, incident reports, and data reporting to meet audit and funding requirements.
- Coordinate client intake and assessments in collaboration with relevant stakeholders.
- Facilitate referrals to internal and external services to ensure holistic client care.
- Promote a safe and inclusive work environment by adhering to Workplace Health and Safety guidelines and GARC WHS policies.
- Maintain strong communication with clients, families, service providers, and the broader GARC team to ensure consistent and culturally appropriate care.
- Actively participate in quality improvement initiatives and continuous professional development.
- Participate in internal and external audits, reviews, and service evaluations.
- Undertake other duties as required by the Regional Manager – Aged Care and Disability Services.
Key Selection Criteria
Qualifications and Experience
- Minimum Certificate III in Individual Support (Aged and/or Disability), or willingness to obtain within the first 3 months of employment.
- Demonstrated experience in aged care or disability service coordination and case management.
- Experience in supervising and supporting a team in a community services setting.
- Current NT manual C Class Driver’s Licence.
- NDIS Worker Screening Check, or the ability to obtain and maintain
Knowledge and Skills
- Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards.
- Proficient in care planning, client documentation, and data collection for funding and compliance purposes.
- Effective communication, conflict resolution, and stakeholder engagement skills.
- High-level time management and organisational abilities with the capacity to work independently.
- Proficiency in Microsoft Office Suite and ability to use client management systems.

Desirable Selection Criteria
- Additional qualifications in Community Services, Aged Care, Disability, or Leadership.
- First Aid and CPR certification, or willingness to obtain.
- Food safety certification, or willingness to obtain.
- Previous experience working in remote Aboriginal communities.
- Familiarity with electronic care systems or aged care/disability reporting tools.
Conditions of Employment
- The role may require travel within the region and occasional out-of-hours work for community events or consultations.
